There was concurrence among reviewers that the study was a valuable addition to the literature and the reviewers were uniformly positive. Uniformly positive reviews are unusual. One review reflects some of the issues related to double-blind review. The reviewer was knowledgeable about the relevant literature and urged the authors to more fully use specific references. These references were written by the authors of the current article. Single-blind review (authors’ identity disclosed to the reviewer) would have allowed the reviewer to address the issue more directly. In addition, it illustrates the give-and-take often involved in finalizing a manuscript for publication.
